A medicine list for your physician: making it clear and useful
No independent medical review is claimed. UKRMED editorial policy
Before a physician appointment, it helps to prepare a list of everything you take. Include not only prescription medicines but also over-the-counter products, vitamins and supplements. You do not need to decide beforehand which are important enough: the doctor will assess the information.
For each product, write its name, dose and how you actually use it. If packages look similar or names are hard to read, bring them or take clear photographs. Mark medicines taken regularly or occasionally, and those you recently stopped, including the reason.
Separately mention unwanted reactions, difficulty following a schedule or unclear advice from different specialists. An honest conversation is not about blame. It helps establish the real situation and agree on a plan you can understand and follow in everyday life.
After the appointment, update the list and note the date of the change. If the doctor adjusts treatment, clarify what is being replaced and what stays the same. Do not stop or add products yourself simply to shorten the list before your next visit: it should reflect facts, not an idealised schedule.
